Deep Vein Thrombosis Adult Multiprofessional Critical Care Review, 2007.
Deep vein thrombosis (DVT) occurs in the presence of venous stasis, such as venous valvular insufficiency, right-sided heart failure, the postoperative period, and prolonged bed rest or immobilization. Abdominal operations requiring general anesthesia of >30 minutes places the patient at risk for venous thromboembolism (VTE). Orthopedic surgery of the lower extremity has long been recognized as one of the greatest risk factors for VTE. More than 50% of patients undergoing elective total hip replacement and knee replacement surgery without prophylaxis develop DVT, and >90% of proximal thrombi in hip replacement patients occur on the operated side. The first controlled trials demonstrating that anticoagulant prophylaxis reduced pulmonary embolism deaths were carried out in this high-risk group, so patients with fractures of the pelvis, hip, or femur are of significant historical relevance.
